Content on this Site is for educational and informational purposes only and is not medical advice. It does not replace an in-person consultation with a qualified healthcare professional. Individual results vary; before-and-after photos and testimonials are illustrative, not guarantees. If you have a medical emergency, call 911.

3) No doctor–patient relationship via the Site

Communicating with us through the Site, forms, email, direct messages, or social platforms does not create a doctor–patient relationship. A doctor–patient relationship is formed only after you are accepted as a patient and consent to treatment.

4) Personal information we collect via the Site

When you use our Contact page or other forms, we collect information you provide, which may include: first and last name, email, phone, the services you’re interested in, how you heard about us, and your message. We use this data to respond to inquiries, schedule consultations, provide information about services, and for internal administrative purposes (e.g., quality, safety, and training). 

We may also receive information if you engage with our social media links/embeds (e.g., Instagram) or visit external partners (e.g., “Shop Skin” hosted off-site). Those third parties may collect data under their own policies.

• Shop Skin (skinbetter store): an external ecommerce site.

• Instagram and other social platforms.

• Payment/financing information pages and/or providers.
  These third parties operate under their own terms and privacy policies. We are not responsible for their content or practices.

7) Communications (email, SMS, phone, DMs)

By providing contact details, you consent to us contacting you about appointments, services, and updates via email, phone, text message, or direct message. Message/data rates may apply. You can opt out of non-transactional marketing at any time (e.g., by replying STOP to texts or using email unsubscribe links). Operational messages (e.g., appointment scheduling) may still be sent.

8) User conduct & acceptable use

All Site content—including text, graphics, logos, photos, videos, and layouts—is owned by or licensed to Adlakha Plastic Surgery and protected by applicable laws. Patient images are published with consent and may not be copied, redistributed, or used without our prior written permission.

10) Pricing, services & external scheduling/commerce

Services, descriptions, and availability shown on the Site are subject to change without notice. Quoted prices (if any) are not binding until confirmed in an in-person consultation and formal treatment plan. Any ecommerce purchases (e.g., skincare) made through external sites are governed by those sites’ terms.

11) Financing & payment plans

Information about financing or payment plans may appear on the Site and may link to third-party providers. All financing decisions, terms, and disclosures are between you and the external provider and governed by that provider’s terms.

12) HIPAA & health information

We maintain medical records and protected health information (“PHI”) for our patients in accordance with HIPAA and applicable law. Our Notice of Privacy Practices explains how we use and disclose PHI and your rights regarding PHI. That notice is provided to patients separately and is not replaced by these Terms.

13) Children’s privacy

This Site is not intended for children under 13 years of age. We do not knowingly collect personal information from children under 13 via the Site.

14) Security & retention

We implement reasonable technical and organizational measures to protect information processed on or through the Site. No method of transmission or storage is 100% secure. We retain information for as long as necessary for the purposes described, to comply with legal obligations, resolve disputes, and enforce our agreements.

15) Disclaimers & limitation of liability

The Site and its content are provided “as is” and “as available.” To the fullest extent permitted by law, we disclaim all warranties (express or implied) regarding the Site and its content. To the maximum extent allowed by law, we shall not be liable for indirect, incidental, special, consequential, or punitive damages, or for any loss of data, profits, or revenue, arising from or related to your use of the Site.

These Terms are governed by the laws of the State of Ohio, without regard to conflict-of-law principles. You agree to the exclusive jurisdiction and venue of the state and federal courts located in Stark County, Ohio for disputes arising from or related to the Site or these Terms.

We may update these Terms from time to time. Changes take effect upon posting to the Site with the “Last updated” date above. Your continued use of the Site after changes are posted means you accept the revised Terms.
